Complete the Renewal Application: Patients must complete the appropriate Adult Patient Application or Minor Application and select “Renewal” as the application type.

Healthcare Provider Documentation (If Required): Patients under 21 must provide a recommendation from a licensed healthcare practitioner. Patients 21 and older may renew without a provider recommendation, subject to current District regulations.

Gather Required Documents: Applicants must provide proof of D.C. residency, a current passport-style photograph (2” x 2”), a copy of a government-issued photo ID, and income verification if applying for a reduced fee. Caregiver documentation must be included if applicable.
